| Hospice Care |
A dedicated team — physicians, nurses, home health aides, social workers, spiritual care counselors and volunteers — develops a care plan that is tailored to the patient’s individual needs. The hospice team regularly visits to assess the patient and offer support to caregivers.
Family Hospice and Palliative Care helps to keep patients comfortable,
surrounded by loved ones — wherever that may be — home, skilled
nursing and assisted living facility, hospital, or inpatient hospice facility.
The patient and the family are at the center of decisions about care. In accordance
with a patient’s wishes and needs, all services are coordinated by the
hospice team. Patients and families are never alone. Hospice staff is on-call
24 hours a day, seven days a week.
Family Hospice and Palliative Care supports individual choices, caring with
dignity, and respect. When life expectancy is measured in months rather than
years, Family Hospice and Palliative Care is available as soon as you need
us, for as long as you need us. |
